It's a YoyoJam Lyn Fury with Belldandy sidecaps. I bought the yoyo, took out the original caps, printed these (i had to crop and resize first), cut them out, and put them in.
This can be done with any YoyoJam yoyo that uses paper caps held in with clear plastic caps, like the Lyn Fury, Kick Side, Hitman, Dark Magic, X-Convict, and Mini-Mo Tu.
